Copeptin proAVP shows a significant correlation with vasopressin, it is stable ex vivo and performs precise measurement. In addition, it is easy to manage with results available in less than one hour. Copeptin is a reliable surrogate marker of vasopressin (AVP/ADH) Copeptin helps to rule out AMI with Troponin.

PY - 2020/4. Y1 - 2020/4. N2 - Objective: Antidiuretic hormone (ADH) is involved in the response to stress and in depression and anxiety.

Copeptin may have a role during the intracellular processing of provasopressin, possibly contributing to correct structural formation of the precursor, which leads to efficient proteolytic maturation (7). Background: Copeptin, the C-terminal moiety of provasopressin, is cosecreted with vasopressin. Copeptin may be a useful parameter to characterize disorders of water homeostasis and can be readily measured in plasma or serum. However, it is unknown to date how circulating copeptin and vasopressin levels correlate at different plasma osmolalites. ADH, produced in the supraoptic and paraventricular locations of the hypothalamus, acts on the collecting tubules of the kidney to cause increase in permeability to water and urea. ADH release is triggered by a number of both osmotic and nonosmotic stimuli. A recent quantitative analysis of 50 patients with SIADH, which underwent serial measurements of copeptin during hypertonic saline infusion, revealed five distinct types of osmoregulatory defect ("type A to E") without affiliation to specific underlying diseases.

Copeptin, the C-terminal portion of provasopressin, is a 39-amino acid glycopeptide of unknown function . Copeptin may have a role during the intracellular processing of provasopressin, possibly contributing to correct structural formation of the precursor, which leads to efficient proteolytic maturation .

Measurement of ADH is difficult in part because of its short half-life. Copeptin is more stable and can be measured by immunoassay. It can therefore be reliably used as a surrogate marker for ADH. Copeptin is a glycopeptide of 39 amino acid residues that is cleaved from a precursor of ADH and secreted to the circulation in an equimolar manner with ADH. Determination of the copeptin level is quick and reliable, and improves the diagnostics of osmotic disorders. The copeptin assay may be a useful alternative to direct measurement of AVP concentration. Copeptin is stable for days after blood withdrawal and can be quickly and easily measured. The copeptin assay may be a useful alternative to direct measurement of AVP concentration.
